DISH takes fraud and identity theft issues very seriously and is committed to protecting consumers against all forms of identity theft.

Inquiry Removal Request

To submit a request for removal of an inquiry from your credit file, please download and complete the form below.
(If you are filing an identity theft claim, you do not need to submit a separate inquiry removal request.)

As of June 2020, DISH Network does not post credit impacting inquiries to a consumer's credit file when inquiring for DISH satellite television services. These inquiries are considered soft and are not disputable but are available on your credit file as a reference.

Identity Theft Claims

In the unfortunate event that your name and/or Social Security Number has been used to set up a DISH account without your authorization, you can use the information on this page to initiate an identity theft claim. If you have not spoken to a DISH fraud agent yet, please contact us.

DISH requires three documents to validate a claim of identity theft.

A valid U.S. Government issued ID

  • Passport

All copies must be valid and 100% legible. Please black out the identification number on your saved document.

Proof of Residency

Please ensure the date on the proof of residency matches the date the DISH account was created. This date should have been provided by the DISH fraud agent.

Due to COVID19 there will be a delay in processing identity theft claims and inquiry removal claims submitted via mail. Please use the upload tool above for all claims to ensure timely processing.

What Happens Next?

Upon receipt of the documentation, it can take up to two weeks to process the claim, at which point you will be contacted by the DISH Identity Theft Team by mail, phone, or email. Additional documents may be required to complete your claim.

To protect yourself from further instances of identity theft, DISH suggests that you contact the credit bureaus to set up a 7-year fraud alert OR credit freeze.
